How to record family history

The Family History window lets you record which family members have a history of specific conditions. This information appears in the patient's Clinical Summary tab and is available throughout the patient's record.

Conditions with recorded history are highlighted in red. Tick Show only items with history in the top right of the window to filter the list to these conditions only.

Record family history

  1. In the Clinical Summary tab, click Family Hx.
  2. Click Edit.
  3. Tick the checkbox for each family member known to have a particular condition.
  4. In the Detail tab, optionally select an age bracket to indicate when the family member was diagnosed.

  1. Enter any General Notes or Factor Notes in the fields at the bottom of the window.
  2. Click Save.

General Notes display every time the Family History window is opened. Factor Notes are linked to a specific condition and appear only when that condition is selected.


Add a new condition to the list

If a condition is not listed in the Family History window, it can be added via the List Manager.

Conditions added using this workflow are available for all patients in HealthTrack, not just the current patient.

  1. In the Clinical Summary tab, click Family Hx.
  2. Click Add Factor  in the bottom left of the window.
  3. In the HMS List Manager window, click Edit List.
  4. Click Add Item.
  5. Enter the condition name in the Value field.
  6. Click Save Item.
  7. Repeat steps 4-6 for each additional condition.
  8. Click Save List.
  9. Click Close to return to the Family History window.
  10. Restart HealthTrack for the new conditions to appear in the list.
