Admitting a Patient for Hospital Stays

What are Hospital Admissions?

An admission records a patient's hospital stay. It moves through a simple lifecycle:

  1. Pre-Admitted: the stay is planned, but the patient has not arrived.
  2. Admitted: the patient is in hospital.
  3. Discharged: the stay has ended. HealthTrack records who discharged the patient and when.

When you admit a patient, HealthTrack creates an episode that groups the records for that stay, such as clinical records and theatre visits. Upon discharging the patient, the episode can be closed.

Admit a Patient

An admission can be accessed in a number of ways:

  • While on the patient's record, navigate to Hospital > Admissions and select the Pre-Admit or Admit button.
  • Navigate to Hospital > Admissions Manager and select the Pre-Admit or Admission button.
    • You can also right-click on a record and select Pre-Admit or Admission.

From the Admission / Discharge / Transfers screen that opens, use the buttons to select the Admit or Pre-Admit buttons in the top left and record necessary details on the Admission tab and select Save when all necessary data is entered.

HealthTrack checks required fields before it saves. Depending on your site's configuration, these can include the mental health legal status, admission class, discharge intention, source of referral, funded by and accommodation fields. If the save is blocked, look for a required field that is still empty.

Transfer a Patient

To record a patient's transfer to another ward or facility, you should not modify the data on the Admission tab. Instead, use the Transfer tab to record this data to ensure there is an accurate history of the patient throughout their hospital stay.

  1. Open the patient's admission.
  2. Go to the Transfers tab.
  3. Select Edit.
  4. Click in the grid to add a new Transfer row.
  5. Add any required details.
  6. Select Save.

Conditions & Procedures

Condition and Procedure codes for the stay are entered on the admission's Coding tab.

  1. Navigate to the Coding tab in the patient's admission.
  2. Select Edit to make changes to the admission.
  3. Select Edit in the Conditions or Procedures sections.
  4. Select any necessary codes to be added.
  5. Select OK when all codes have been selected.
  6. Select Save when you are finished recording the necessary data.

Discharge a Patient

When the patient's hospital stay ends, they can be discharged. HealthTrack stamps the discharge with your name and the date. You can also close the episode at the same time, which marks the grouping for that stay as finished.

  1. Open the patient's admission.
  2. Select Discharge at the top-left.
  3. Enter required details in the Discharge tab.
  4. If needed, Print Discharge Summary.
  5. Select Save.
  6. Specify if the episode should be closed, then select OK in the Discharge prompt.
  7. Status will now appear as Discharged in the Admissions Manager.

Hospital Admission Integrations

Some HealthTrack systems receive admission messages from other hospital systems. Where this is set up, an admission can be created or updated automatically without anyone at your site entering it. If an admission appears or changes and no one on your team did it, the feed is the likely source. Contact HealthTrack support if a feed is creating admissions incorrectly.
